Sliding Fee Discount Program
At Comprehensive Care Center of SWLA, we are committed to ensuring that quality healthcare is accessible to all individuals, regardless of their ability to pay.
Through our Sliding Fee Discount Program (SFDP), we offer services at reduced cost to patients who are uninsured or underinsured, based on household income and family size. Discounts are determined using the most current Federal Poverty Guidelines (FPL) issued by the U.S. Department of Health and Human Services.
We proudly serve all patients, regardless of insurance status or ability to pay.
How The Program Works
Patients with incomes at or below 200% of the Federal Poverty Guidelines may qualify for discounted services
Patients at or below 100% of the Federal Poverty Guidelines may receive services at no cost (nominal fee may apply depending on service type)
Patients above 200% of the Federal Poverty Guidelines may still receive care but will be responsible for full charges
Discounts apply to eligible services within our scope of care
All patients are expected to provide insurance information (if available) , and we will bill third-party payers prior to applying any sliding fee discounts, in accordance with federal requirements.
Eligibility
Eligibility for the Sliding Fee Discount Program is based on:
Total household income
Family/household size
Insurance status (uninsured or underinsured)
Patients are asked to provide documentation such as:
Recent pay stubs
Tax returns
Social Security or disability statements
Unemployment documentation
Self-declaration of income (if no other documentation is available)
Eligibility must be renewed at least annually or sooner if there are changes in income or household size.

The Comprehensive Care Center of SW LA is dedicated to reducing barriers to care and improving the health of our community. The Sliding Fee Discount Program ensures that no patient is denied access to services due to inability to pay.
*The Comprehensive Care Center of SW LA also offers assistance for patients living with HIV through the Ryan White Program. Individuals at or below 400% of the Federal Poverty Guidelines qualify for Ryan White services.
