HIV CASE MANAGEMENT SERVICES

Case management is at the heart of our client services. Our approach to case management is based on the principles of trauma-informed care, utilizing motivational interviewing techniques that are delivered in a judgment-free space that is welcoming and accepting. We are mindful of the social determinants that affect health, the injustice of social inequities, and acknowledge the negative and harmful experiences that people may have experienced while interacting with healthcare entities and providers. The goal of case management is to assist clients with connecting to the resources they need to improve their health outcomes and quality of life.
Once a qualifying client has completed an intake, Case Managers may assist them with supportive services including:

  • Healthcare enrollment and linkage to care

  • Identifying and reducing barriers to care

  • Providing outpatient mental health and substance use support

  • Facilitating referrals to other community resources

  • Job Readiness and Seeking Employment

  • Coordinating transportation for medical and support services appointments

  • Food Pantry Assistance

  • Housing Assistance

  • Referrals to Legal Assistance
